No sleep, Reflux or spoiled?

Our 9 month old has reflux and we have had sleep issues his whole life.  He is now on his third medication, prevacid, and his sleep seems to be getting worse!  How do we know when his night waking is due to reflux/pain versus his being spoiled?  He was up 8 times in 11 hours one time screaming for almost two hours straight last night in our arms, but did not have any obvious reflux sounds.  He is so tired all the time averaging only 7-10 hours of sleep in a 24 hour period.  We really are wondering what we can do for him?

I am in the same situation with my son.  He's 3-1/2 months old and was diagnosed with acid reflux when he was 4 days old.  He was on Zantac, up until a recent visit to the ER, where doctors discovered he has laryngomalacia and that his larynx was irritated from acid, so they switched his meds to Prevacid.  He seemed to be okay for the first few week but now, 3 weeks after starting the medicine, his sleeping is worse, he is constipated, gassy, and extremely uncomfortable.  We are having such a hard consoling him and most days he just cries himself to sleep.  I stopped giving him the medicine just yesterday and I have a calll into the ENT doctor who prescribed the medicine but I have not heard back from them yet. 

 

What medicines has your son been on?

He started on Zantac, then we tried Prilosec, which constipated him and made him miserable.  We went back on Zantac, much higher dose, which took the edge off for a little while ten stopped working.  So now we are on the Prevacid, with a call into our GI doc for the next step, which is a sleep study.

I just got a call back from the ENT doctor at Children's saying to go back to the Zantac, a higher dose, 3 times a day.  Hopefully this will work as I think we're running out of options, and we all need sleep.  Let me know what you doctor recommends.

How is your son doing?  Have you been to the doctors for a sleep study?  I just wondered what came of that. 

 

Connor is back on Zantac now and his sleeplessness continues, at least its better than the constant crying.  I wonder if the medicine could be causing the sleeplessness.  Of course, every time I mention it to the doctor, they say his sleeplessness is just colic and he'll grow out of it.  Yesterday, he was up at 5 am, slept from 7 to 8, then was up pretty much the rest of the afternoon, haven taken only two 30-40 minute cat naps, then at 4:00, he napped for 2-1/2 hours and last night he woke up about 4 times in 7 hours. 

 

I keep thinking his sleeping habits will get better as he gets older but its been 4 mos already and he still continues to have such a hard time staying asleep.  I don't want him to spend the night in the hospital, but I think he needs to have a sleep study done as well to check for apnea, so I was just wondering what happened with your son.

We are scheduled for an upper endoscopy, a 24 hr PH probe and a sleep study on 12/1.  So we have two more weeks to wait for the tests.  I have wondered about the meds making it worse too?  I guess we will find out because he needs to stop his prevacid 7 days before the tests and the zantac 3 days before.  He is still having his wakeful, screaming nights.  Last night he was up 7 times between 10pm & 7am.  I am hoping that we get some real answers from the tests!

I am sorry to hear that your son has to go through all of that testing but hopefully it will provide you with a better understanding of what's going on so he can start to feel better and not have to wake up screaming.  I am interested to find out how your son sleeps when he is off the meds.  We have a follow-up appointment with the ENT doctor this week, but I'm not looking forward to the trip to our local Children's Hospital, as my son hates being in his car seat and screams either the whole there or the whole way home.  Needless to say, we don't go very many places with him.   

 

I can't imagine having to go through this for 5 more months...my husband and I are exhausted now.  It's hard to work full-time with a baby who keeps you up at night.

The tests went as well as could be expected.  He didn't sleep any better off the meds and we could feel him refluxing!!  We haven't gotten any results yet, waiting and hoping for them every day!  He is back on Prevacid and Zantac, and still up 4-7 times a night.  Hopefully things will work for Connor.

I am sorry to hear that Colby did not sleep any better off the meds.  Have you gotten any results back?  I hope the test results will give you some answers as to what is keeping him from sleeping. 

 

Connor's sleep study is scheduled for Feb 7th, I hope he actually sleeps enough to complete the test.

Hi all. My son was a very bad sleeper with extreme gas and "spitting up". More like vomiting, about 30 times a day.

What I learned was that gas is caused by undigested food entering into the large intestine where bacteria feeds on it and causes gas. Please try a probiotic in your infants bottle, I used CULTURELLE as it worked great and helped his eczema. You can use it safely in each bottle if you wish.

All it contains is the good bacteria that the infants body needs to help digest food properly and fight infection.

Concerned Mom,

Is that probiotic a prescription medicine or can you buy it over the counter?  Because of the laryngomalacia, I suspect that my son will continue to be gassy until he "outgrows" these conditions and I'm concerned about what is going to happen when he starts eating solids.

You can purchase any probiotic at a nutrional store or Vitamin Shoppe. Look for one that has no preservatives or any other allergic component. I have one that is free of dairy, corn, milk, or soy.  I am no familiar with laryngomalacia so I don't feel confident to comment on that. I don't believe the probiotic would effect it in any way as probiotics are safe and are just the same thing we produce in our gut naturally.

Thanks, I am going to ask the doctor about it next week.  I don't think it could hurt to give it to him, especially if it helps heal his stomach, maybe we can wean him off of his medicine sooner than later.  

 

Laryngomalacia occurs when the flap of skin that covers the larynx is floppy and does not stay closed like it should.  You may have heard of "stridor", it's pretty common in babies with reflux.  My son's "inspiratory stridor" is caused by the laryngomalacia so when he breathes in, it makes a high-pitched noise (kind of like a wheezing).  That muscle is similar to the flap of skin that closes in the stomach (lower esophageal sphincter) to prevent babies from spitting up.  Laryngomalacia is one of those conditions that a child should outgrow as the muscles strengthen.

Well I'm glad to hear that he should out grow it! That's great news. Some doctors don't like probiotics because they are available over the counter and there are no FDA studies on it, so you might get some unfavorable feedback. That's the experience I had anyway. But then I questioned why we were giving Prevacid to a 3 month when no studies have been conducted on children under the age of 1..... needless to say the doctor was stumpped and did not answer me.

I went to another physician who treated my son for "reflux" and cured him in two days. He also gave a calcium supplement to strengthen the lower esophageal sphincter, the little flap you were speaking of. Not sure if this method could help your son, but worth some investigation....

Lisa,

I am aware of studies showing that motility medications increase LES pressure and strengthen contractions of the esophagus. I have not heard of doctors recommending calcium supplements to strengthen the LES. What research studies have you heard about?

Jan, you can find some studies on homepathic websites. The doctor I saw in Manhattan is an MD, Board Certified, but thinks outside the box and wants to heal you not just put a bandaid on the problem. My son is one study an proof enough that it works. Plus it also has a calming effect since we had noticed my son was a bit edgy. Since calcium is totally natural and found in mother nature, it's better to try this than a prescription med with a laundry list of side effects.

Hello Lisa,
I am glad you were able to find a practitioner to help you. I have heard several success stories like yours. A toddler I know was having terrible respiratory complications from GERD and a great deal of work was done by a homeopathic doctor to look at dietary triggers. He was able to get off of his GI meds with a VERY strict diet.

 

The hard part is finding a practitioner with knowledge and experience working with an infant or young child.

 

If you know of any specific studies or web links I should look at re calcium, please pass this information along. I am always learning something new.

Our oldest daughter was around 9 months when we put her on Prevacid.  It got worse.....we found out about 2 months later from the pharmacist that there is a form of lactose in the coating of the medication.  They had our daughter put on a pharmacist made suspension and it solved the problem.
