Hello, Our 4 month old son is currently on Zantac 15mg. He's been on it for about a week now and we're not seeing any significant improvement. I know a week may be too early to see any change but we're getting nervous and don't want him to end up with esophagitis or something worse. GERD is very new...

Hello Sandeepa, Since your son has been on the Zantac for one week, it might be a good time to check in with the doctor. Let him know what you are seeing at home and give him an update on the feedings.

The good news is most babies do not have severe esophagitis or need surgery. The reflux seems to cause a great deal of discomfort and makes sleeping and feeding almost impossible. With all of that going on, it would make sense that there is severe damage to the esophagus. Adult GERD often results in damage to the esophagus but not infant GERD. The goal of reflux treatment is to reduce pain so a baby can eat, sleep and grow. You are in the most difficult stage of reflux treatment where you and the doctor are still trying to figure out the best approach. You might need to talk to the doctor a lot right now until you both feel that the treatment plan is really working.

I just sent you my phone number in a private email so lets try to talk by phone today.

    Hello Jan,

I'm sorry I have not called you yet. We took the children for a short vacation since our older son had one week off from school. As you can probably guess, I mainly stayed in the hotel room and tried to sleep feed my 4 month old reflix baby. I will try to call you as soon as I can. Right now I have my hands full with trying to feed my son and see a psychologist for my own depression issues.

Here's a brief update about our baby: yesterday he had a follow-up with his GI specialist. The specialist put him on Prevacid solutabs (I had asked about Prilosec suspension but he said many pharmacies do not carry it). He gave me enough office samples of Prevacid to last two weeks and then scheduled an upper endoscopy for two weeks from now. He also encouraged me to try to feed my baby solids while awake. He wants my son to take the Zantac and Prevacid both. Is that common? I am starting to become convinced that my baby may have developed a feeding aversion so I insisted on getting some phone numbers of pediatric feeding rehabilitators and OT and speech therapists. I'll keep updating. Today is day one of the Prevacid and right off the bat I noticed my baby slept a lot after taking it. Not sure if I should worry or not.

    Hi Sandeepa,

First off: yay! you have the right meds now. There are many signs of improvement that the baby will start showing when a drug starts working:

    1. Sleeps well

    2. Back arching reduces and then stops completely

    3. Spit ups happen but are less traumatic for the baby.

    4. Sleep feeding starts become a much smoother even but the baby develops a habit to continue with it.

    5. Slowly (after two weeks of PPI therapy) begin solids. Swallowing solids make the esophagus stronger and the reflux reduces

Do not worry about the side effects of PPI. As worst they are mild to moderate and can be fixed with normal probiotics-- I went to 4 GI's (latest one yesterday) and all of them vouch by it. My newest GI has asked us to add periactin an appetite stimulant and I am asking other moms on my feeding support group about their experience with it.

How much of solu-tab are you giving your baby? The drs samples are better than those that pharmacy will give you. Pharmacy samples get stuck in the syringe so I would suggest taking a tiny drop of water in a spoon, gently make the solutab damp and soggy, pick it on your finger and put it into the baby's inner cheek.

Do keep us posted and good luck with everything. Do not worry about G-tube, fundo, or esophagitis. I have been there and your phase is the most worrisome phase. Once the PPI takes full effect, you will realize that things move very smoothly. Please check my sharepost on medications and how to tell when they are working. I have talked to several drs, GI's, moms, and therapists and that's more of a review.
