I agree that I tend to do a much better job with keeping my kids' records than my own. I kind of keep mine in my head but recently I realized I needed more for my docs. Luckily, I've been asking for copies of lab, x-ray, ultrasound, etc results for years. I just scanned all of these in and now keep them in a medical file on the computer. For me, this does two things. I have easy access to them and won't lose them and when I go to a new doc I can pick which ones to print out to take.

 

I like that you are suggesting a short log though - I just went to a medical conference and the docs there admittted that while they want the info they need something they can scan very quickly and get the most information from. What I got out of this is either just document a couple of days or document everything but then write up a short summary for the doctor.
