I have had similar issues due to GERD.  Alcohol and cigarettes can be GERD triggers so it is always possible that could be your problem as well. That having been said I would think it would be wise to rule out other causes for chest pain, especially with your history of smoking.  For some, especially women, heart problems can also present with GI symptoms such as nausea and vomiting.  Chest pain should never be taken lightly so it may be time to make another appointment with your doctor!
