We started it 3 days ago and my son has started screaming in pain.  He's spit up most of his life, but never was in this much pain.  I can hear the reflux and see milk in his nose.  I never was this severe before.

Oh, that is not good! Please check in with the doctor about the new symptoms you are seeing. Let me know what you find out. I do hear parents report new symptoms from medication. Sometimes it takes a few days for the body to adjust to the medication. Other times, it is a sign that another treatment needs to be tried.
