Friday, February 11, 2011 Becky asks

Q: My baby is 12 weeks and at her 2 month dr appt she was diagnosed with GERD and placed on Prilosec. She now only will eat a couple minutes on each breast and she cries during feeding.

Pulls off the breast and then refuses to latch back on. I have taken her back to the dr, twice and she is not concerned because baby is gaining weight. Although it seems to be getting worse it is every feeding that occurs and the amount she eats is getting less and time between feedings is getting longer. Help!

Hello Becky, Your baby is communicating that something is wrong. It is challenging for parents and doctors to interpret these cries and figure out what is wrong. Certainly reflux can cause discomfort during and after feedings. Some babies begin to associate feeding with pain and try to avoid eating. There are many other reasons babies refuse to nurse or take a bottle:

infection or illness:

Thrush

sore throat

ear infection

urinary tract infection

there may be digestive discomfort from:

food allergy or intolerance

milk soy protein intolerance

yeast overgrowth

poor suck causing too much air ingestion

foremilk/hindmilk inbalance

constipation

I wonder if you have consulted a lactation specialist to discuss your diet, her latch and feeding and the schedule/frequency of feedings.

If her feeding is worse since she has started the medication, talk to the doctor about the dosage and timing of the medication and whether another medication may be needed. Some doctors stop reflux medication altogether if the baby is the same on and off medication.

I have given you lots of ideas here-please let me know if you need more details or links to information.

It is important to follow up with the doctor frequently so you can go over the treatment plan and make any adjustments.

Take care, Jan

Thank you. I am meeting with a lactation consultant tomorrow. We will see what she says. I have cut all milk out of my diet because it did seem like that was bothering her. I have begun to pump a little milk before I feed her because it seemed like she was getting impatient to wait for the milk to come and that has seem to help a little the last couple of feedings.

Maybe it would be best to get a second opinion? Every doctor is different and has their ways of handling things. :]
