You are right that the dark urine is not explained by the reflux. The lower back pain could be any number of things from your kidneys to referred pain from your stomach to muscle pain. The sore in your nose actually could be due to acid but you are right the third one this year should be checked out.
Unfortunately, there's no way to be diagnosed over the internet. If you aren't getting the answers you need from your doctor perhaps it's time to seek a second opinion.
