Saturday, October 11, 2008 Sophia's mom asks

Q: 8 Wk old was prescribed Prevacid after being diagnosed with thrush and treated unsuccesfully

My 11wk old daughter was prescribed Prevacid at 8 wks. At first she was diagnosed with thrush and prescribed Diflucan which made her projectile vomit, then the pediatrician prescribed (Nystatin)unsure of the spelling sorry. At first that wasnt working either she was vomitting on that also. We were then told to spread it aroung her mouth on a cotton swab for two weeks. That solution seemed to help but after the two weeks of reccommended dosage there was no improvement. The doctor then told us to stop all medication for one week and if there was no improvement then to call him back.  There was no improvement so we made an appointment and that is when the acid reflux was diagnosed. At first we were told to put cereal in all of her bottles and given a prescription for the Prevacid but advised to hold off on filling it. After a week of just giving her cereal in each bottle, one night I noticed that she sounded very congested and seemed to be having trouble breathing when laying flat on her back. I thought it was something I could get with the suction bulb but I couldn't get it. Being a first time mom I called the pediatricians office to see what to do. They then told me that it wasn't really congestion it was the acid reflux acting up and to go ahead and fill the prescription for Prevacid 15mg solutabs. We filled the RX and tried dissolving it in water and giving it to her in a bottle. She drank all the water but all the particles of the pill were still in the bottle. We have finally figured out a way to get it down, we just break the pill into very small pieces and put it in her cheeks and she seems to like the taste and takes it fine.  What I am trying to get at here is that she has been on the Prevacid for almost 4 weeks now with no improvement in the extreme white coating on her tongue. There has been improvement in the "congestion" that I had noticed before but she still spits up from time to time and I just wonder if this(Prevacid) is the right solution for us or not?! Does anyone have a remedy to get this coating off of her tongue aslo? She sticks her tongue out constantly like it is bothering her. Suggestions Please!!

Dear Sophia's Mom,

You have had quite an eventful 11 weeks. I hope things calm down very, very soon.

I am not aware of a connection between thrush and GERD. Do you think she has thrush and GERD? It sounds like the medication and thickened feeds have helped with the vomiting. I wonder if you need to have the pediatrician look in her mouth and see if the white coating is thrush or something else.

It might have seemed like the medication caused the projectile vomiting. Sometimes babies with GERD seem fine for the first few weeks and then start showing symptoms. I am sure you are not eager to try the thrush medications again after your experience with them the first time around.

Let me know what you find out!

Hi Sophia's mom,

It seems as though you are having quite a rough time. I hope things get better for you soon! If in fact your daughter does have thrush, and it sounds like she might, the Prevacid is not going to help resolve that. Thrush is a fungal infection and needs to be treated with an antifungal medication, such as the Nystatin and diflucan that you used previously. If your daughter has GERD, the Prevacid may help, and she may be able to better tolerate the medications that made her vomit before. Either way, she probably needs to see a pediatrician again in order to figure out what's going on in her mouth, as well as the best way to treat it. Good luck with everything!

My daughter had both acid reflux, and thrush within a couple weeks of life. I believe that the thrush makes her acid reflux worse. I went through two orders of the yellow stuff, that she hated, and it didn't work. Then they finally gave me Fluconazole. Which worked great. Her thrush was cleared up for a while for a couple months. But is now back. I am starting to think she gets the thrush infection so easily because she spits up in her mouth, and it stays moist, more so then children. She is on Similac Sensitive RS....she is also lactose intolerant. It is thickened with rice, and lactose free. And takes Prevacid twice a day. I am starting to think her system needs Acidophilus to help her digestive tract, and to help keep yeast at bay.

I am going to try home remedies of apple cider vinegar, yogurt, and possible powder Acidophilus, if the yogurt doesn't workout (considering she is lactose intolerant, and she is just starting on rice cerel). I am now going to wash all her stuff with some vinegar in the rinse cycle to hopefully kill anything residing on her cloths, and start disinfecting all her stuff. I hate to think of buying new bottles again. But if she is not heading in the right direction shortly I will be taking her to the doctors again, because I think the thrush is painful for her, though it isn't for most babies. My last baby didn't have any thrush, or any problems like this, just lactose intolerant. I am sorry you are having such a hard time, first time around.
