I get severe pain in my upper stomach and in my back could this be more then Reflux??

I get severe pain in the upper part of my stomach right where the ribs come apart and along with this I have pain in my back at the same spot and sweat all over. I have been taking Nexium for years and when I have these attacks I take it and it doesn't work. The pain is so severe that at times I have had to go to the emergency room. The only time the pains weekens for a bit is when I hold my breath. Obviously I can't do that for long. Any ideas?? Is it really just Reflux or maybe something else??
