Q: USE OF ANTACID FOR OVER 14 YEARS - IS IT LINKED TO IBS-D

Dear Sir:

I have been on antacid pantaprozole (pantodac 40) since 1998, after being diagonsed with duodenal ulcer and hiatus hernia (5mm). Very difficult to stay without it. Strictly avoiding spicy food. Severe complain of IBS-D for more than a decade. Are the two interlinked. What best alternate.

Irritable Bowel Disease is a possible side effect of Proton Pump Inhibitors but they are the most effective drugs for reducing stomach acid - which will have caused your duodenal ulcer.

There are many different PPI's and, although they are all similar, some people tolerate some better than others. You may wish to try omeprazole or lansoprazole instead for a while to see.

Histamine H2 Reaction Antagonsists - eg Zantac (ranitidine), also reduce stomach acid by a different mechanism, though not so effectively.

However, they may also cause IBD as a side effect but you could try them for a while to see if they are better tolerated.
