Q: What dose is too high for a 3 month old on metoclopramide? My daughter weighs 12 lbs.

She was dx with acid reflux with she was 4 weeks old.  She has had an upper GI and everything (anatomically was fine) as well as an ultrasound to rule out pyloric stenosis. The dosage has recently increased from .5 to .7 mls every 8 hours.  She is also on Zantac .7 mls every 8 hours.  My question is:  Is this dosage to high for a 3 month old?  What should I be looking for in regards to side effects.  I have noticed the tongue thrusting, but don't know if it is because of the medication or if it a baby thing.  I would like to ween her off this medication, but don't know if that is a good idea. Any info you can provide will be greatly appreciated. Thank-you,Concerned Mommy

 

Case History:

My daughter was born at 34 wks 5 days. I developed preclampsia.   She weight 4 lbs 15 oz and was in NICU for a week.  She was provided O2 for a few days.  A GI was referred when taking her to the hosptial secondary to , what we know now as a bad reflux episode.  The milk escaped from her mouth and nose causing her to choke.  This has happened on several occasions.  After seeing the GI he ordered an ultrasound to rule out pyloric stenosis.  Ultrasound came back with no pyloric stenosis.  Then after several more scary episodes he addmitted her to the hosptial for an upper GI which came back fine.  While at the hosptial we found out she has a milk protein allergy and was  switched from neosure to alimentum with rice cereal.  After 2 weeks on the new formula, my daughter continued to have scary episodes and was then switched to neosure.  I then switched her rice cereal to organic brown rice cereal which contains no soy.  She has done so much better.  She still has a scary episode every now and then, but not as bad.  Sorry this is so long.

It sounds like you and your doctor are working together to find the best treatment plan for Jade. So far, you are making progress. I am so glad the scary breathing episodes are decreasing. My daughter had breathing problems from reflux and I hardly slept at all worrying about her!

You may have heard about the new warnings on Metoclopramide on this site and in the press. It is important to be aware of the possible side effects, especially hand/arm tremors and seizure like movements. The most important part of the new warning is new advice on only using Metaclopramide for short term use. As you follow up with the pediatrician and GI doctor, be sure to ask if Jade's medications need to be changed, decreased or stopped. It is important to decrease Metoclopramide slowly to avoid additional side effects.

Be sure to ask the doctor about other treatments for reflux. Some doctors recommend elevating the sleeping surface to improve symptoms. It may also help to keep her upright after a feeding and for play. I imagine you are already doing this. It is a lot of work to keep a baby elevated but often this is an important "medicine" too!
