Did your primary care doctor or a GI specialist prescribe this? You are right that there are many potentially serious side effects that go along with this drug especially the longer it's taken.
Honestly, I would consider surgery before I would consider Metoclopramide. Of course, surgery is not without risks but the risks of Metoclopramide especially that they are potentially irreversible after stopping the medication are just too high for my comfort. On the other hand, I am controlled okay with Pepcid. If I wasn't I would consider PPIs again. If those didn't work I would consult a surgeon. If that wasn't an option then there's a good chance I would consider the Metoclopramide.
The risks of not adequately treating are high as well. Kind of a damned either way situation......and then picking the lesser of the evils.
