Prevacid and Prilosec are both Proton Pump Inhibitors and therefore they are supposed to work the same. But, they are different PPIs and for some people one of them works better than another. Also, OTC strengths differ from prescription strengths so you cannot compare 30 mg capsule of prescription Prevacid to 20 mg capsule of OTC Prilosec or the new OTC Prevacid.

Most doctors I've heard speak on it or I've talked to say that it is very dependent on the person and they prefer to go with the least expensive option first whether that be prescription or OTC generics and go from there.
