Q: Hyperactivity...mental vs. physical

Is ADHD ONLY the physical form of ADD?  I mean is ADD  mental hyperactivity and not physical? Or is ADHD mental hyperactivity and physical when ADD is only distractability? Another way of asking I guess would be...is distractability due to hyperactive thoughts that keep my wheels spinning which prevents me from being able to focus and concentrate?
