I understand your fear. Five feels like a very early age to start medication, doesn't it? If your son is truly ADHD and has been diagnosed by a psychologist through testing, then you may want to try alternative approaches such as diet changes, holistic use of herbal remedies and/or therapy with someone who specializes in attention problems. You can find a specialist in your area through Psychology Today.
I am also an extremely strong proponent of Neurofeedback. I have used it very successfully in ADD/HD clients, both in children and adults. I would encourage you to learn more about, if possible. You can read "ADD: The 20-Hour Solution" by Steinberg and Othmer. I have seen students who fit your daughter's description, turn completely around and become honor students by doing the training. If you decide to go that route, it requires a 40 session committment. Find someone who has a background in understanding the brain. I try to discourage people from going to a neurofeedback therapist who decided to "try" it out instead of really knowing how powerful it is in changing how the brain functions.
The problem with ADD is that mood disorders and low self esteem is usually part of it, and a good therapist needs to address that as well. On the other hand, if, after trying some alternative measures you aren't seeing the results you would like, then you might consider medication.

As a teen who has been an meds sience 3rd grade (which I know isn't 5 years old) and I think it is very helpful for school. I know I am not an expert nor have I been in this situation of giving a my kid meds ( I don't even have kids) however, I would say wait untill he goes to school. When your son gets to a grade with big tests, like state tests, I know that in my state extra time is given to children with things like ADHD. You would proboally want to talk to a doctor about this nt a teen but I know if I had been diagnosed before I had started school that taking meds would be pointless. Also I was diagnosed with ADD which is ADHD w/o hyperactive. So if your son is very energetic the medicne might help relax him but I am not postiive about that.
Also my doctor has talked to me about seeing someone who would teach me ways to control my attention without taking meds but only started to talk about after I turned 13 and really understood ADD.
