When should ADHD be treated w/medication as oppossed to behavorial modification?

My son has been on several meds [i.e., adderral, concerta, focalin xr] and was taken off because of mania [couldn't get thoughts out of his head (e.g., touch this, touch that, etc. never anything aggressive) plus, he had no appetite (e.g., 57 lbs at age 11)].   He is VERY IMPULSIVE. ...
