Sundowning seems to be a prevelant and important issue. Having a plan for daily acitivites etc., that revolve around the way patients respond at later points in the day seems vital to having a successful day.

My father-in-law does have this problem. Sometimes he does not recognize that he is home. One day after he went out and saw the doctor in the morning, he did not feel like he was home. At 4pm, he started saying he wanted to go home and tried to get up from his couch. He needed to walk to the kitchen and sat down to have dinner "in the restaurant", and then walked back to the couch to finish this "trip". I think he also did not understand the "steps". He likes to say too many steps to take the bath and etc. Maybe he just wanted to complete the steps from the doctor's office at sunset. The caregiver tried to "feed" him to distract him. We just took him to the kitchen table and had our early supper and "went home" after the quick supper - he kept saying "I want to finish this meal ASAP" to go home.
