Q: How do you stage atypical, early onset alzheimer's with severe physical impairment?

My mother is 60 and has been diagnosed with atypical(cortical basal). early onset alzheimer's. It was difficult getting the correct diagnosis (took 5 years to rule out parkinson's and a stroke).  Finally she saw a specialist and had a PET scan and MRI done (which also found lesions on her spine--which I am sure is adding to the impairment)   I have only noticed mild cognitive impairment and some mood changes, however her physical ability has declined severely---she falls backward, has a shuffle walk, her body freezes--she has become so impaired that she is now having bladder issues, needs a walker and help getting up.  She is in chronic pain and is physically going down hill fast.  QUESTION:  What stage is she in, should I be preparing for the worst?  Have you ever heard of alzheimer's that presents as more physical than mental initially"

Hi Janey

 

Because your mother has an atypical type of Alzheimer's, I think the stage she is in will have little relevance or be that helpful to you. Stages of Alzheimer's are based on usual types of Alzheimer's and can be helpful for caregivers to give guidance on care plans and care delivery. In your mother's case it sounds as though her physical disabilities will take priority over any cognitive ones.

 

Her doctor is the best person to seek advice from and is in the best position to give you an idea about how her disease will progress. Your love and support are going to be very important. So is effective pain control and maximizing her mobility and enjoyment of life. It is tragic that you are all having to deal with this when she is so young.

 

Contact with your local chapter of the Alzheimer's Association may also be important to your mother, you and her extended family. They can offer support and put you in touch with other families coping with early onset Alzheimer's.
