My Mom always wants to "Go Home" How do I understand what she is really trying to say?

My Mom has Alzheimers, she was diagnosed in 2004, I read that you usually Die anywhere from 4 to 6 years after being diagnosed. If this is true My Mom is in her last stages, I sometimes find myself just as confused as she is, I dont know what she is thinking, I dont know how I can understand how...
