My Grandmother has a friend living with her who can no longer take care of him self.

My Grandmother lives in NYC and is 85 years old and has a family friend living with her who is 82 years old and can no longer take care of himself he is incontinent he does not bathe my grandma has to help him with every thing she is to old to help him she can barely help herself she wants him put into a nursing home where he will be better taken care of.

This sounds like a good option to me. Maybe you need to call in social services to do a welfare check on them, and then SS could help place him.
