You posted your question as a sharepost, a blog, and you might want to copy and paste it as a question. You will get more responses that way. My first impression is that you are in love and afraid that your girlfriend will forget you. You seem to be describing normal anxiety and sadness that would be expected of anyone going through what you are talking about. You didn't mention if you have had anxiety issues in the past, but there doesn't appear to be anything unusual in what you are describing.
