Your questions are all good ones but way out of the range of expertise of nonmedical providers. If you have a complicated medical presentation and are taking medications with competing actions you need someone who is very skilled at prescribing. If your psychiatrist doesn't want to manage all of your psychoactive meds then you may want to find one who will. There are many ways to approach your problems but you need someone who is very experienced. If you live in a large metro area or have access to a university affiliated teaching hospital you may be able to find a psychopharmacologist if you can't find a psychiatrist who is willing to work with all of the meds.   Rick Wirtz   Rick Wirtz is a Psychologist, not a psychiatrist or medical doctor.
