Got my eval from U of M

It is long!!  I suppose that's good, considering I was there 4 hours or so.  I also received a several page personality profile last week.  Says a lot of things I disagree with, got a couple of factual things wrong.  But the essence of it is about what I thought it would be.  For those who are interested in the process, I'll touch on the categories that were addressed in the evaluation:

 

Blah blah blah about age, race, etc.

 

Chief complaint.

 

M-strides outcome (that stupid computerized test that asks how many night you had a hard time sleeping out of the last 2 weeks, etc)

 

Family history or mental illness.

 

Personal history.  They cover stuff like if you were a difficult baby, premature, how much your parents were around growing up, your gpa in high school and college, any counseling history, etc.

 

Employment history.

 

Religious preference.

 

Military history.

 

Social history.  Family you live with, whether you have a strong social network, etc.

 

Sexual and marital history.

 

Current medications.

 

Allergies.

 

Herbal supplements.

 

Past trials of psychiatric medicines.  What you took, why, and what effects it had.

 

Sleep medications.  Ditto above.

 

Past medical history.

 

Past surgical history.

 

Current review of symptoms.  Got a cold?  They want to know!

 

Vital signs, weight, etc.  Do you feel warm or cold today?  Are you a night owl?  When was your last period, and how long was it?  Do you get more manic or depressed depending on the weather? Do you get bad PMS?

 

Substance abuse history.

 

Psychiatric history.  Counseling, meds, were you ever anorexic or bulimic?  Anxiety? OCD?  All sorts of stuff like that.

 

History of presenting illness.  How long ya been feeling crappy?  Describe.

 

Mental status examination.  Oddly, this includes how you dress and are groomed.  How you respond to answers, whether you fidget.  Sarcasm is noted.  Evaluates insight and judgment, etc.

 

Suicide risk.  Evaluates suicide risk factors.

 

Formulation.  This is like a summary.

 

Positive prognostic factors.  This is things that are going for you, ie good marriage, don't use drugs or alcohol, etc. 

 

Negative prognostic factors.  Things that are not going for you.  Mine includes "unstable temperament and difficulty maintaining employment".

 

DSM-IV diagnoses:  It goes Axis 1-5, which you can google to learn about the different elements. 

 

Disposition and plan.  This is the part where they recommend specific medications, therapy, etc.  Mine includes a plan A, plan B, and also a specific note about avoiding something I told them my primary medical doc suggested.  THIS is the part you suffer through the 4 hours for.

Hi Freeda, How are you doing? I'm glad you got all of your eval. info back. Are you doing it just as your actual evaluation, or with the bipolar study they have going there? I know it takes a long time, but in some ways it's fun. I got my personality eval. back recently, and it was almost totally accurate. It was interesting. Well be glad at least all that is finished now!! Take care. Keep me posted.

Just evaluation... As for the study, I'm not sure if that's the big questionnaire I did?  They sent me that big personality profile thing earlier, I thought that was from the little extra thing they asked me to do while I was waiting for my appointment.

 

My psychologist got his copy today, and called me immediately -- Love him!  So I'm going to see him next week, but he just wanted to make sure that I was okay with the report and whatnot.  I kind of crabbed to him about some of it, but in the end I felt pretty good.

 

So looks like I'm going to start Lithium.  I think it's a good time to start it, because my daughter's gone for the summer and I'm done with classes for now, so I have very few stressors while I get adjusted to it.  I'm actually feeling pretty good lately. :)  Hope you are too!

HI Freeda, I'm glad you like your therapist. Let me know how it goes with the lithium. I've never taken it before, but I now it's usually the first med they look into . I hope it goes well for you. Are you seeing the therapist here at the U of M, or where you live? I know you mentioned it's quite a drive for you to come to the U of M. Keep me posted on how you feel on the lithium. Take care.

I see people locally.  I just went to U of M for the broad overview, recommended plan of treatment, etc.  ;)
