Scroll down to bottom. Sign in for free (you do not have to check the boxes at the bottom) & there is both a questionnaire & then an 'interview' with pictures that you can do with your child. Printing the summaries is free, but scoring sheets cost $25 USD. I would just print out the free summaries & bring to your doctor.
