Outrageous insurance company position regarding claim for double mastectomy

My insurance company (Oxford) decided that the max. amount allocated for my double mastectomy is $ 662.00 . Since I went out of network, they would in turn pay 70% of $ 662.00. I appealed this decision with a letter from my surgeon explaining in great detail the procedure performed. The appeal was denied. I now have another opportunity to appeal... Read more Chevron
