Q: What is Metastatic Adenocarcinoma of the breast, N1, recurrence?

I have had a recurrence of breast cancer. It was only stage 1 B and I had a double mastectomy.  Previously I had had a lumpectomy and also radiation treatment.  The cancer came back in the same location, only this time, after the double mastectomy, it appeared in the lymph nodes.  My path report says, "Metatstatic Adenocarcinoma, N1, 3 out of 8 lymph nodes." Also, it mentions that N1 , or could be greater than N1.  What are the usual treatment options and prognosis for this diagnosis of a breast cancer recurrence?  It is Her2nu negative and Progesterone poss. and Estrogen poss.  Could you please help me?  I am confused as to how in the world it could have come back after a double mastectomy in the first place??  Thank you for your time.  LP

Hi - I'm a bit confused here - you had a lumpectomy/radiation; then a double mastectomy; and after that, metastatic adenocarcinoma? If so, it sounds like there was still some cancer left after the double - which is possible, because even a double mastectomy can never remove 100% of your breast tissue; and, the cancer could have already spread into the chest wall or skin. And that cancer traveled to your lymph nodes. N1 means 1 or more nodes are affected. HER2-neu negative, ER/PR positive is good; it means you can possibly benefit by taking hormone therapy drugs. I'd assume your treatment will be much the same as it is for invasive breast cancer - surgery, chemo, perhaps radiation. But, best to ask your oncologist about the specifics, OK? Good luck - PJH
