It could be dehydration. It could be eczema. Although unlikely, it also could be a form of cancer called Paget's Disease of the Breast, which has a scaly rash on the nipple as its main symptom. Please don't panic because I doubt that Paget's would get better with moisturizer. However, a doctor can diagnose that exact cause of the rash and recommend the best cream. If you had just noticed this rash, I'd say use unscented skin care products, a good moisturizer, and other common sense home remedies for dry skin. However, since this has been a problem since the fall, letting a doctor take a look is a good idea.
