I feel a weird lump, have pain and see an indent in my left breast. should i be concerned?

I had a mammogram about 4 years ago, and it showed nothing. The doctor told me it was from too much caffeine. It that was the case, why don't i have same symptoms in my right breast? Since then, the pain feels worse and now there is an indent next to the nipple when i raise up my left arm.…
