Q: I am worried about inflammatory breast cancer and need advice on symptoms and diagnosis

 

2 months ago I developed pain in right upper outer quadrant of breast near the armpit.  I noticed changes in the breast- intermittent swelling (not related to menstrual cycle), blotchy appearance, development of small broken capillaries.  Pain became severe and constant in same location quickly-- has not subsided at all.  Pain sometimes shoots into breast like pain with engorgement after giving birth (i have two kids).  There is intermittent swelling into the right arm.  Now, there are changes in the skin over the breast that do not appear on the other breast- a kind of scaly/waxy looking patches around nipple area.  I have been seeing drs. in speciality clinic for last month and a half, who don't seem concerned.  They did finally take two biopsy samples, one from above nipple where there was some slight discoloration and one from blotchy red area with broken capillaries.  Got results two weeks later, after I tracked down the report-- first biopsy "sparse perivascular lymphocytes present" which were stated to be within normal and second biopsy was LOST-- the one taken from the red area.  Dr. doesn't want to do repeat because "first one was fine" and "I don't think its cancer"-- she says its mastalgia and I should take evening primrose oil and cut out caffeine.  Am I being paranoid?  The breast continues to look weirder and weirder and there has been absolutely no relief from the pain-- been taking 1000 mg of Naprosyn a day for 2 months and Vicodin as needed- which dr. cut off yesterday saying it will make pain worse.  I need some advice-- I'm worried this delay may be severely jeopardizing my health!

I am 38 years old and have a family history of cancer (not breast)-- mom died at 39 from colon cancer (died same year as diagnosis), dad was diagnosed with adenocarcinoma of the lung last year.  Additionally, my maternal aunt had ovarian cancer (survived) and my paternal grandmother had adenocarcinoma in the ovaries (died same year).

Your use of the word intermittent to describe the swelling doesn't sound like typical IBC because usually cancer symptoms stay the same or get worse.  However, your overall situation is getting worse, and you are dealing with doctors who lost a biopsy sample and don't seem concerned about it.  I think it would be entirely reasonable to see a different doctor and start over in your search for a diagnosis.  Maybe your doctors are absolutely correct in their diagnosis, but losing a biopsy sample doesn't inspire confidence.
