Davison, it sounds like you have two separate questions: follow-up health care, and economic help.  Five years after cancer treatment, many oncologists will turn a patient's health care back over to the primary care doctor depending on the details of the person's health.  So you don't necessarily need to still be seeing the cancer doctor, but you do need regular check-ups by a doctor who knows your cancer history, and, of course, if you still have a breast, you need regular mammograms.

There is no specific disability for cancer survivors once they have finished treatment.  If you have a specific problem like lymphedema related to your breast cancer that keeps you from doing the kind of work you did before cancer, then an occupational therapist should be able to help you find work that you can do with your current limitations.  The person at DHS who told you that you are no longer eligible for sick pay should have set you up with a job coach or other person who could help you find new work that you are able to do.  I would suggest going back to that person for help in contacting local resources in your community who can help you find work and an affordable doctor.  I hope you soon find both.
