Q: If not a breast infection is it IBC?

A few weeks ago I started having tenderness & achy pains in my right breast.  I felt a lump where it was most tender & painful.  I am 40yrs old and my youngest child is 3 and I didn't breastfeed her so I figured it would be odd for me to have mastitis.  I woke up the next day and it was swollen & the skin was reddish/purple, like a bruise right where the lump is.  My lymph nodes in my neck and under my arm were also swollen and tender.  That was on a Saturday so I waited until Monday and called my GYN.  The breast swelling had gone down by Monday but the lump was still there and still tender.  I had never had a mammogram so he immediately sent me for one followed by an ultrasound of the breast.  A cyst was ruled out but the radiologist didn't know what it was so I had a CORE biopsy a few days later.  It came back benign and indicated "inflammation"...possible mastitis.  It was sent on to another lab for further testing.  In the meantime, I developed very swollen achy lymph nodes on the entire right side of my body.  My throat developed a white pus-like coating and I had alot of throat pain when I swallowed.  The dr did a strep test which was negative and my blood work all came back normal.  I was put on Rifampin and Cleocin for a week, which will be finished in a few more days.  The second report came basically the same as the first indicating "inflammation" but not abcess.  My throat is better with the antibiotics however the breast lump is still there and I anticipate that it won't be going away.  My dr. wants me to have it taken out completely if it's there after I finish the medicine.  Should I be worried about IBC?  Thank you.

Meg, although mastitis is more common in nursing women, it can happen in non-lactating women.  It's good that you have been proactive with your doctors, and it sounds like they are following up appropriately with the biopsy and imaging tests.  With the sore throat and other generalized symptoms of infection, it doesn't sound like typical IBC.  

Did the core biopsy also sample the skin?  For a definitive biopsy to diagnose IBC, the pathologist needs to find cancer cells in the lymph vessels of the skin of the breast.  Since core biopsies only sample a small area, they are better for diagnosing solid lumps than cancers like IBC that can be thinly spread out over a large area.  Your doctor's plan to remove the lump completely sounds reasonable to me.  At that time the pathologist would do more thorough checking to get you a definitive diagnose.

I'm not a doctor, so I don't know all the possibilities, but there are other causes of breast inflammation besides infection or IBC.  I hope you soon find out what is causing your symptoms.  I know it's hard to avoid worry when something is wrong with your breast.
