All the women in my family have fibtystic breat disease. I desperatly need a mammorgram (I haven't had one in 3 yrs due to loss of insurance & employment.) I have sent 2 proof of required affidavits to the Korman Center in Boca Raton, FL (where I reside), but they keep sending the same applications, despite my faxing them twice w/ the documentation they requested. Can you please help me? Tahnnk you.
