Smoking increases blood pressure and can slow healing after surgery and, of course, it is a cancer risk.  I don't know if there are any risks to stopping smoking suddenly while on cancer treatment, so your sister should talk to the doctor about the best way for her to stop.  Since smoking is a stress reliever for many people, stopping when under the stress from cancer may be especially difficult for your sister.  On the other hand, cancer often motivates people to take better care of their bodies.  The sooner your sister can quit safely, the better her health will be.
