Q: Need help to make decision about chemo

 I am 49 years old. This past January I had surgery for DCIS. I was diagnosed with IDC after having a lumplectomy, early stage T1a, Nuclear grade high (3), Her2 + (3+), er-pr-breast cancer. The margins were not clear, so I had a reexcision and a Sentinal Node Biopsy. (Partial Mastectomy right breast). The margins still were not clear, and more DCIS was found in other parts of my breast, so I was told I needed a mastectomy. The good news was the Lynph Nodes were negative.

 

With my risk factors, I opted for a bilateral. On March 10, 2010 I had a double mastectomy and expanders put in for reconstruction.

 

I just saw my Oncologist on March 19th. She informed me that she recommends chemo and Heceptin and told me I am a good candidate for a Phase II Trial of Adjuvant Paclitaxel (Taxol) and Trastuzumab (Herceptin) for Node Neg Her2- Positive Breast Cancer. The other option if I choose not to go with the trial would be "TCH" Taxotere, Cacbablalin, and Herceptin (every 3 wks x6 (18weeks or 4.5 months) and Herceptin every 3 weeks for 7.5 months after that. The trial would be:  Taxol & Herceptin given IV every week for 12 weeks. Then I would then received Herceptin every 3 weeks or weekly for 4o weeks. I was told the trial would be easier to tolerate, and not as toxic. What are some quetions I should ask my doctor on April 2 when I see her again to help me make an informed decision.

Hi - You might want to read our post on deciding if a clinical trial is right for you; and our clinical trial FAQS. I've never been faced with this decision, but if I were going to get some facts straight, they'd include the expected results of the trial - are researchers expecting the same or better results as the current standard of care, with fewer side effects? I'd ask specifically the expected side effects of each type of chemo, current standard vs. clinical trial, and what percentage of patients would be expected to have which side effects (so that you can get as close as possible to gauging what side effects each option would give). And I'd ask what the possible lasting effects are of each (lasting effects = long-term after-effects.) Also, be sure to ask payment questions - is the trial covered by insurance, or by the institution sponsoring the trial? Good luck with this decision - PJH
