Since your sister has recently been breastfeeding, this could well be an infection as PJ suggests. Swelling, pain, and heat can also be symptoms of a rare form of breast cancer called inflammatory breast cancer, so please urge your sister not to wait on seeing a doctor because of her financial situation. If the list of resources PJ gave you doesn't include a place near your sister's home, have her call her local Komen for the Cure affiliate or the nearest American Cancer Society office about resources for her. Most communities also have free or reduced cost clinics.
