Q: My implant falls to the side when i lay down on my back - should I have additional surgery to tighte

Typically your implants should only move slightly within their pockets, which should be just a tad larger than the implants themself.  If your implants are shifting too much it's likely because the pockets are too large.  There is no real way to force implants to stay in place, except by creating pockets to hold them.  Consult with your plastic surgeon and see what he recommends doing to fix it.  You can try gently "pushing" the implant into place, but that won't provide a permanent solution.  I do beleive surgery is the only fix.

Thank you for your time and answer.  My thought was just your answer.  Do you know if another drain will be there when I wake up --- they are uncomfortable.

Most likely yes.  I've had 9 breast surgeries, and each time I walked away with drains.  It's better to be safe and prevent infection, but it would be up to your doctor.
