Pulomonary Emboli & Pulmonary Hypertension: My chest won't let go of me!

I Really need answers that make sense to me please. Persisant pain, breathing problems, central chest tightness, shoulder pains, midback pains, and speaking problems have been issues since PE & Pulmonary Hypertension diagnosis and 3 hospital stays beginning - 6/27/11.  Coumadin therapeutic (INR level just reached). But this condition still has laid me out an average of 3x/week and does not seem to be getting better. It's not as bad as first time, but very close. Keep talking myself out of E.R. trips- I don't want to leave/scare kids. Is DVT in right leg feeding problems? Thrombophilia center says: "it wil subside. But these symptoms can go on and on." Is that true? The problems seem too severe to dismiss. I cannot fully use my lungs to support speaking in a normal tone, If this is normal, please tell me what can I do to make it better besides just treating symptoms with morphine prescribed for LB & neck injury pain of which center is aware I am doing this. I am used to dealing with pain issues. But, this one scares me and just doesn't seem "right."  ~Thank you for any advice.
