how to interpret the value of the cholesterol level?

I am hoping if someone can help me with these questions.  We are having trouble understanding these numbers. 

 

One is this.  My friend has taken cholesterol level checked without fasting and the value was 278mg/dl.  Trigryceride was also higer than referenced value.  I forgot the number.  She ate 2 slices of toast with a half teaspoon butter, and 2 cups of coffee and maybe a piece of chocolate before this testing.  The night before she had a bunch of asparagas with soy sauce for her dinner.  She eats at least 4 eggs a week.  She jogs about half an hour almost every day. 

In the past, she has always way below of 200 for the fasting cholesterol level. 

So my question is, is this value expected since she did not fast, or a bit /a lot higher? 

 

Second, another friend's cholestrol level was 256mg/dl recently and the value has not changed for last 5years.  She has been on cholestrol pills for last 15years.  however, her LDL went over about 60 mg/dl than the reference value.  (I don't remember what the exact number. )  it was within normal value last year.

My question is, could this dramatic change in one item happen this way?
