need a plan

Just had my physical for this year....not good. Last year cholesterol was at 216 A little high, but nothing like what I was told this year. Triglycerides at 586 cholesterol at 287 HDL at 42. High Triglycerides

I had several stents. I also Had an open heart surgery in 2003 , since then I have. I have high triglycerides (389) despite taking both Zetia and Tricor as well as Omega-3 oil from flax seed & fish oil as well as 4000 IU of SLO Niacin.
