Has anyone had Triglyceride numbers like mine? (1909) please help

need a plan

Just had my physical for this year....not good. Last year cholesterol was at 216 A little high, but nothing like what I was told this year. Triglycerides at 586 cholesterol at 287 HDL at 42.
