Please, any suggestions?

By cyberchar Tuesday, March 04, 2008

I've been all around this site, but I don't think I've been in the right place at the right time or something. I am struggling to find something to ask my doctor to give me for pain management. In a way, I am lucky. I have diagnosis of both rheumatoid arthritis and degenerative disc disease, so there is no problem with being prescribed narcotics like some of you with those horrid soft tissue injuries that hurt like the devil but never show up on x-ray. My problem is different from that. Back in July, while getting standard blood work done for a procedure which inserts a device in your spine that causes  electrical jolts that are supposed to take away the pain.

Well as I have written before, and I apologize for duplicating myself, it turned out my kidneys were at only 20% effeciency and my liver is being destroyed by cirrhosis of the liver. I was heading for a morphine pump until I overdosed on the morphine push button machine because my liver & kidneys just don't rid my body of the narcotics at any specified rate.

So since July I have struggled on Methadone, 10 mg 4x a day and Morphine, fast acting, 15 mg 4x a day, along with Xanax and an anti-depressant.  The doctor who admitted me from the clinic where I have been going for over 3 years knows that it was his mistake for not checking my chart and calling in orders by phone until my respiration got down to 3 per minute and my pulse 30. So now he has decided that I need to "lower my expectations" about pain management and get by on just a 30% reduction. Before this incident, I was taking 3 80 mg oxycontin, morphine, 15 mg 4x a day, and had a fentanyl patch of 100 ml. I know this was a LOT of narcotics but dog gone it, I was in a LOT of pain. I couldn't even make my Malto-Meal because I couldn't stand at the stove for the 3 minutes required to make it.

Well, I think the Narcan they gave me lowered my resistance to pain, and I was doing well on the 4 Methadone & 4 Morphine. Well, it's not anymore. I have been up vacuuming every morning at 2:30 am, if I fix something to eat I am too tired to eat it, and my grocery shopping is limited to about 20 minutes. It's a heck of a way to live. If the Good Lord is only going to give me 5 more years or so, as the doctor told me, I don't want to spend it suffering and not even able to pick up my 3 yr old granddaughter to put her in my chair with me.

Can anyone please suggest something? I have an appointment in a couple of days with my PA whom was responsible for getting me my disability, and who just wants me to be comfortable. The physician who is head of the clinic once offered to pull a tooth for me that was horribly swollen because I had no insurance waiting for my settlement.

Any thoughts would be so appreciated. I can't take aspirin or tylenol due to my liver disease which was just Hepatitis C in remission from a $12 tatoo until I went on Enbrel. Thanks to all, Char

Hello Char

It's nice to meet you and I'm glad you found us.

I'm really sorry for all your pain and really hope that your doctors are able to get you in a more comfortable place with your disabilities.

I think that's Deb's suggestion for the stimulator was a good one.  It's really hard to tell about meds, as with your kidney problem probably complicates things badly.   Since your not new to pain, I'm sure you already have tried nearly every technique & treatment available, so I'll not insult you and tell you to relax & take deep breathes or to try yoga or something.  But if you've not tried message or a stretching routine, you may want to try one.  I've personally found that going for a walk outside can just make my head clearer and give me a break from the constant drain that pain puts on us.  Some people find that stretching & breathing gives them a great deal of relief, but for me, the relief is very tempoary and when pain is flaring it does absolutely nothing!

I really hope someone comes along with a great new idea for you, but when you talk to your doctor, I would tell him exactly what your feeling and don't hold back about what you want out of the time you have.

Gentle Hugs

Betty

preally appreciate everyone taking time to write such considerate posts. In fact, it was the spinal stimulator that I was getting the medical & psychological tests for when they discovered my kidney problem. First, because the psychologist wasn't there, they tried to put me into the stress unit. When I explained again the reason for my appointment with the psychologist, they did say that they thought I wasn't appropriate for lock-down. Then we got that settled, started to leave when the ER doc stopped me, started an IV, and took me off over half my medications due to my kidneys. My cardiac doc told my son that had he not done that, I would have been dead in another couple of days.

So, because of that, along with the mess about the morphine pump, I haven't even heard back from my pain management doc. I was really hurt that after he did a microsurgery on both sides of my back to drill off some of the bone spurs and heaven knows what else, he didn't come when my doctor called for a consultation from a pain management doctor. Their office is on the 3rd floor, I was on the 2nd floor, yet both of the other doctors in the practice came down to check on me and not MY pain management doctor. Needless to say, I am not quite as thrilled about going back to him. The one pain doctor, who set up the morphine machine, told me that he would gladly put in a morphine pump when I was out of the hospital. But now I'm afraid THAT would backfire as well.

Anyway, I finally decided yesterday to call my doc and tell her how badly I was hurting. She told me to forget the other doctor's 20% reduction in pain (I noticed I typed 30% in my initial post), the five years he's given me to live, and has prescribed a low dosage fentanyl patch which won't react so much with my liver.

I think one of the scariest times was when I was hurting so badly although I was on oxycontin, morphine, a 100 mg patch, Lyrica, and heaven knows what else, and my son told me that there was no where else to go, that I was at the point where there was nothing stronger to take. I pray I'm never in that position again.

Thanks again for taking the time to write. You don't realize how much it means, living alone in a high rise full of elderly & handicapped people, no car, with only a couple loud mouth cockatiels to keep me company.

All my best to all,

Char

I have Hep C and am in constant pain...not to mention the fatigue factor. I find it difficult to work at least 2 out of the scheduled work days per week. My dr. put me on Tramadol, generic for Ultram, and it isn't doing ANYTHING  to help my pain. I know most pain meds go through the liver, so I am curious if anyone knows of a pain med that could help????Please????? I'm desparate here. Also, have no insurance, life is very frustrating right now.
