Do I have TMJ?

I have had chronic neck muscle pain for a couple years that always hurts into my jaw, forehead and eye muscles.  I've been going through spells of dizziness, blurred vision, fatigue, pain in lower back, facial tightness, and a few times... tingling pinkies.  No one knows what is wrong with me!  Please help!!
