Q: what treatment should i take for a 8 months old tmj pain and dislocation

i am having a pain in my tmj and tireness of my facial muscles.my OPG was taken .what treatment can i take

Hello,

I developed a tmj disorder after a serious car accident 14 years ago.  To date I have tried dozens of pain-control meds (limited success), and have had 3 surgeries.  The last, a menisectomy to remove the disk, helped with jaw opening, but increased pain.  I have had surgeries to remove bone spurs, also with no pain decrease.

For about 10 years I have had Botox injections into the tmj.  They seem to help a little.  My most helpful things though are sticking to a soft food diet, limiting talking on the telephone (unless I am using a headset), using ice (freeze styrofoam cups full of water) or heat (heat has always worked best for me). I think that it's important too to look at the cause of your tmj pain - are you grinding or clenching, or is there a post-traumatic injury?  Mouth appliances or physio seem to help some people.

And, most importantly, try to relax!  Aromatherapy and keeping busy seem to help me.

I have been to some of the " supposedly " best TMJ doctors in the nation. I deal with TMJ as a side issue, but several of them wanted to do surgeries. I happened to come in contact with a person who had the surgery from one of the doctors that had recommended it, and she had all of her disc to collapse as I have.  She had lived in more pain after the surgery than before.  I have avoided surgery, and I use an appliance for 24 hours of the day.  It helps with a litttle control.  Mine is so bad that I have trouble sleeping because of the pain, and it even causes my ears to hurt.  The doctors said they had never heard this, but I have to rotate my position very frequently as I try to get what sleep that I can.

If your appliance is not properly fitted, it can cause even more pain.  Check out your area and talk with as many people to see if they can recommend a good doctor.  I drive almost 300 miles, but it is worth it. I'm not suggesting that advoiding surgery is the best treatment, but I have since talked with many patients who have had it and were considerably in more pain after the surgery than before.  I also got on the internet and researched for the best doctors in addition to recommendations from my physicians. I was diagnosed with RSD as a result of one of my consultations.  This had been completely overlooked by all of my prior consultations.  I was able to get help for a short while, and my doctor died. Then I had to start over again, and it was not easy to locate a new one. That is the reason for the long commute.
