Of course, I can't say for sure, but Dilaudid shouldn't cause kidney damage, especially if she didn't have any prior kidney problems.  More than 95% of Dilaudid is metabolized in the liver, with only a small amount left unchanged and excreted in the urine.    I'm sorry to hear your wife is having such a difficult time.  I do hope you're able to get some answers and help soon.
