What is the best way to prevent/treat bone disease when diagnosed with stage 3 kidney disease?

I have chronic pain that gets worse when I eat milk products. I was diagnosed with Polycystic Kidney Disease in 1988. In Nov. 2008 I was down graded to stage 3 chronic kidney disease. I was diagnosed with pre osteoporosis in 2007. I am premenopausal and taking .625 mg Premarin.
