This is not my area of expertise but from what I've found, there are two important factors:

 

1.  It must be medically necessary, not just for cosmetic purposes.

2.  Since the states administer Medicaid, it depends on whether your state covers gastric bypass surgery.

 

Here is an interesting document from the National Health Law Program that discusses this subject:  Medicaid Coverage of Gastric Bypass Surgery  The last page of the document lists which states covered gastric bypass surgery as of Sept. 2004. 

 

Also, try asking your question on our Obesity site – MyObesityConnection.com – as they should have better and more up-to-date information than I do.
