It seems that there are two stages for the condition:   Stage 1, without central sensitization. Stage 2, with central sensitization.   Most pusblished work on total reversion of myofascial pain comes from treatment of locallized Stage 1 myofascial pain and trigger points. Stage 2 seems to be more closely related to the chronic condition. Cesar Fernadez-De-Las-Penas and David G. Simons are two very active researchers on the subject. Devin Starlanyl, author of books, is a terrific resource too. Simons co-authored the seminal work on this disease. It does seem that new stuff is coming out, I guess patience is needed. In the interim, moderate, light exercise and avoidance of perpetuating factors (i.e. sedentarism), are the only thing that work for me, with 15 years into the condition.
