what would cause pain on the top of my big toe? it's like i'm being stuck with a needle -

I get this intermittant pain on the top of my big toe just where it meets the nail. I have had xrays, blood tests and a foot doctor examination and he is stumped also. There is no sign of gout or arthritis and externally it looks perfectly normal. Sometimes the pain is so intense...
