What type of specialist manages Opioids, etc?

Mom is in severe pain from a neuromuscular disroder and is in a nursing home. She is on Oxycodone , ativan, and balcofen. The optiates were supposed to be "as needed"-- which has really turned into every four hours. She is also on a slew of other medications (high blood pressure, etc).  We believe is suffering neurotoxity (psychosis unrelated to illness, slowing of breathing). We don't know who to go to for cleaning up this mess. The neurolgist points to the primary doctor who doesn't appear to know much about pharmacology. Any ideas?

I go to a pain management clinic which also houses a pain research institute, to manage all my meds including opiates.  The doctors there are anesthesiologists. Hope this helps.
