Of course, we can't diagnose or treat people online.  Only a licensed physician who can examine you in person and run necessary tests can give you a diagnosis.  If your personal doctor has been unable to help you with your symptoms, it may be time to consider seeing a specialist.  From the symptoms you listed, visiting a rheumatologist might be a good place to start.  Rheumatoligists deal with all kinds of musculoskeletal disorders and most rheumatologists are excellent diagnosticians.

You say you haven't slept well for years.  In that case, it would probably be a good idea for you to talk to your doctor about having a sleep study done.  Our bodies are designed to repair and restore themselves as we sleep.  If we don't get adequate amounts of the good, deep restorative sleep we need, we can develop all sorts of physical problems and illnesses.

I wish you the best as you pursue the cause of your symptoms.
